To join the Sun City Anthem Women's Club, please, print this form and fill out the application. Annual dues are $25. Attach your check (payable to Sun City Anthem Community Association, Inc.), to the application form. Please, note on your check SCAWC dues. Deposit these in the Women's Club check slot inside the Fitness Center at Anthem Center, or mail to the following address: Sun City Anthem Women's Club, 2450 Hampton Road, Henderson NV 89052.
